I developed this template for use in Sussex and it lists every common type of evidence available in West and East Sussex.
Obviously if you are not working in Sussex you will have to maul it about a bit to create your own template, but the idea is that, once created, you can the amended template for any application in your area.
A couple of words of caution
- The document is created in Word. The numbering is automatic. If you edit it in any other word processor the numbering may do wierd things.
- You cannot insert a hyperlink in a paragraph while it is numbered. Delete the paragraph number, insert the hyperlink and then reinstate the paragraph number.
